In 2012, a new medication called Qsymia (with both Adipex and Topamax components) became available for weight loss. It was the first FDA approved weight loss drug in its class. The weight loss pill is a combination of 2 existing drugs: phentermine and topiramate. This combination has been shown to be a good tool for weight loss in addition to a plan that includes dietary change, exercise and behavior modification.

 

Below is a detailed description of each component of this very effective drug combination.

1. Adipex

 (or Phentermine – for generic)

One of the most prescribed weight loss medications (appetite suppressant) in the USA.

 

Adipex is a classic (inexpensive) pill that is FDA approved to be taken daily for  up to 3 months when used as a solo prescription for weight loss. It is now utilized in its lowest dose (~ a quarter to one half pill) for long term use only as combination with topiramate (i.e. Qsymia)

 

Common side effects of Adipex include but not limited to:

  • dry mouth
  • constipation
  • headache
  • insomnia
  • Less common side effects: anxiety, palpitations

Instructions on how to take: Take Adipex in mid-morning every day (take on empty stomach or with a small snack).

To reduce side effects, stay hydrated by drinking plenty of water during the day.

Try starting with ½ tablet and feel free to continue with reduced dose (also effective).

 

Note:

In the United States, it is classified as a Schedule IV controlled substance under the Controlled Substances Act.

2. Topamax

 (or Topiramate – for generic)

Originally an anti-epilepsy and anti-migraine medication. It has been researched and used as part of a new weight loss drug (i.e. Qsymia)

 

The most common side effects of Topamax include but not limited to:

  • transient tingling sensations (in hands/feet…)
  • mouth taste alterations (Coke or soda may seem flat!)
  • small risk of kidney stones (advise drink plenty of water during the day)
  • rare memory problems might be noticed (feels like you are “forgetting a word” in a conversation), which is not usually persistent nor disrupting in general.

We always use lowest dose needed. Hence, most side effects above are mild and tend to improve with continued use.

Pregnancy warning: Risk of fetal toxicity; precautions advised for women of reproductive age. Topiramate may also lessen effectiveness of oral contraception pills.

Instructions on how to take: Start the prescription with once daily at bedtime (or evening) for few days and then increase to full dose (twice daily with or without food).

Note:

Topiramate is also a generic (inexpensive) drug. We have noticed that it is not usually effective by itself for weight loss, but is for sure one of the best treatments when used along with Adipex (phentermine).
